Mobile App Weekly Reflection Form

Learner feedback is an important part of the evaluation process. By using surveys to gather student feedback related to their use of an app, lesson, or website, instructors can make data-informed decisions around whether to continue, expand, or discontinue using a particular resource. Routine use of reflection tools can also support learner engagement by including them in the evaluation process. This Google Forms template allows you to systematically gather user feedback on usability and engagement levels based on their use of assigned or self-selected apps, lessons, or websites.
